Benefit applications and COVID-19
We understand that during COVID-19 it may be more difficult to send your completed form to your employer so they can complete the departmental report before sending your form to us. You can instead just send your SR1, SAR1, SRR1, or SIR1 form directly to us and we’ll contact your employer to get the information we need.

You can use this form if you are the child of someone who passed away while in receipt of a pension from DFRB, DFRDB, or MilitarySuper, or if you have care, control or custody of the child of a deceased pensioner.
DRRP Restoration of reversionary pension benefit application form
This form is to be completed by a person whose widow or widower pension under DFRB or DFRDB was ceased on their re-marriage (prior to 1977) or a person whose pension was cancelled and later only partially restored.

MILSOP Spouse of a deceased pensioner Reversionary pension application form
You can use this form if you are a spouse or partner of someone who passed away while in receipt of a pension from DFRB, DFRDB, or MilitarySuper.
D80 Application for DFRDB spouse, child/student or student pension (including MilitarySuper ancillary benefit)
You should complete this form if you consider that you are an eligible spouse or eligible child of a deceased member of DFRDB.

Grant a third party access to your personal ADF Cover entitlement information.
