Application forms

If you discharge with 20 years or more effective service, please complete the  Application for DFRDB retirement pay, commutation and superannuation productivity (including MilitarySuper ancillary benefit) form.

If you discharge on medical grounds, please complete the Application for invalidity benefit and superannuation productivity (including MilitarySuper ancillary benefit) form.

If you are transferring to another arm of the ADF without a break in full-time service, you are not yet entitled to your DFRDB benefit. Please contact us to discuss your situation.
